The Population Health Planning Guide (Guide) is a collection of information, tools and resources to promote population health initiatives in Washington State. It is managed by the Washington State Department of Health.

The resources on this website can help you:

  • Learn more about population health.
  • Create a plan to improve a health issue in your community.
  • Turn your plan into action.

How To Use This Website

The Guide is organized by specific health focus areas with related resources and strategies, as well as by the key elements of an overall population health approach. The list of health focus areas will continue to expand as updates are made to this site, but right now you can apply this site's population health approach to any issue even if it's not listed.
