This version of the Population Health Planning Guide (Guide) website was moved in its entirety from the Washington State Department of Health’s website. We wanted to make sure the Guide’s resources were available to Portal users as part of the Portal’s launch. In the coming months, we will revise and update the Guide to blend in with its new home on the Hub Resource Portal. We appreciate your patience.

How to use this Website
The Population Health Planning Guide is organized by specific health focus areas with related resources and strategies, as well as by the key elements of an overall population health approach. We will continue to add to the list of health focus areas, but you can apply a population health approach to any issue even if it's not on the list.
