Since its founding, providing care to the “whole person” has been an organizational value at CareSouth Carolina. The practice’s behavioral health care model grew from a patient activation component of a perinatal program and through Wagner’s Chronic Care Model.
In order to successfully and completely integrate behavioral health in the primary care practice, CareSouth Carolina adopted several practice changes:
