Data Profiles

To support the selection of zones, DOH staff will develop data profiles for the highest-scored nominees based on the metrics below, that were identified by the Community Advisory Council with input from the Community Workgroup. Community-based data submitted in the Nomination Form will also be included in the data profile.

These data profiles are meant to provide the Community Advisory Council with a holistic overview of the nominated community alongside their Nomination Form. Data profiles are not scored components of the selection process.
