National database of success stories
The National Association of Chronic Disease Directors (NACDD) has an interactive database and collection of success stories showcasing state and territorial chronic disease prevention and health promotion work. NACDD created the database in 2008 to assist Chronic Disease Units with educating policymakers, funders, and Members on evidence-based and innovative programs they have implemented to reduce the burden of chronic disease in their communities.
Centers for Disease Control and Prevention (CDC): Health System Transformation
The Centers for Disease Control and Prevention (CDC) website contains links to frameworks and resources being used by partners across the county, including Washington.
Two key resources:
- 6/18 Initiative offers proven interventions for costly health conditions, and aligns preventive practices with value based payment and delivery models.
- High Impact in Five Years (HI-5) highlights non clinical strategies with a track record of demonstrating early return and cost effectiveness.
Prevention Institute: Prevention Approach to Medical High Utilization
The Prevention Institute has identified strategies that are specifically connected to reducing high utilization of medical services. This article is especially helpful in addressing health equity.

This site provides access to publications, workshop summaries, and national expertise focused on improving population health.
