Taking proactive steps to address dementia 

The Washington State Department of Health (DOH) Healthy Aging Program, in partnership with the Dementia Action Collaborative of Washington, has awarded grants for brain health and dementia awareness (PDF) to community and faith-based partners. On this page, grant awardees can find documents and resources to support their grant activities and tools to implement the Brain Health & Dementia Awareness in our Communities (BDAC) curriculum.

Grant Templates

Use this Workplan Template (link downloads Word document) to identify the Brain Health and Dementia Awareness activities your organization plans to implement. We will update this page with a Progress Report Template, BDAC Educational Session Report Template, and a Final Narrative Report Template.

 

BDAC Trainer Resources 

Trusted messengers offering BDAC sessions can access downloadable social media graphics and flyers to help promote activities in their communities and copies of the materials needed to facilitate a BDAC session.
