Taking proactive steps to address dementia 

The Washington State Department of Health (DOH) Healthy Aging Program, in partnership with the Dementia Action Collaborative of Washington, has awarded grants to 12 community and faith-based partners for brain health and dementia awareness (PDF). Below are documents and resources to support grant activities and tools to implement the Brain Health & Dementia Awareness in our Communities (BDAC) curriculum.

2026 CBO Awardees and Partners

Use the Awardee & Partner List (PDF) to find 2026 grant awardees and their community partners who can provide information, resources, and services to support your grant deliverables and your community needs. Connect with them to learn about what is available to your community or for help in meeting your grant deliverables. 

Grant Forms

Complete the Invoice Template (fillable form) to submit a request for payment of your completed grant statement of work activities. Required fields and information are highlighted. Save it as an editable document and send it to cbp.invoices@doh.wa.gov.

Use this Grant Activity Report Form to report each educational session or optional activity your organization has completed. 

Use this BDAC Evaluation Survey Form to input BDAC session evaluation survey responses after each training. (Please input only one evaluation response per survey).

BDAC Trainer Resources 

Trusted messengers may use the BDAC Trainer Resources page to find promotional materials, trainer tools to help promote activities in their communities, and copies of the materials needed to facilitate a BDAC session.

Brain Health Resources

Learn how dementia is defined, read about the benefits of early detection, and learn about modifiable risk factors on DOH's webpage about brain health.
