Due March 28: COVID Vaccine Program Planning and Response data surveys
Recently, some local health jurisdictions received two vaccination-related data requests from DOH’s COVID Vaccine Program Planning and Response team: Agricultural and Food Production Workers and People Experiencing Homelessness. The surveys were sent by email to identified LHJ contacts on March 16, 2021. The surveys are open through Monday, March 28.
Data collected in the surveys are reported to state leaders to track progress on vaccinating agricultural and food production workers, and people experiencing homelessness. Those who received the surveys by email can also access them here:

Throughout this survey collection there have been many innovative and thoughtful responses of how to create change and themes of constraints to increase access through community partnerships, increase staffing availability, consistent access locations, and communication avenues and methods that meet audiences where they are at.
