Use this Workplan Template (link downloads Word document) to identify the Brain Health and Dementia Awareness activities your organization plans to implement. We will update this page with a Progress Report Template, BDAC Educational Session Report Template, and a Final Narrative Report Template.

This Invoice Template (Word document) is an optional template you can submit for payment for grant statement of work activities completed. Send as an editable PDF document to tiffany.turner@doh.wa.gov. 

Use the Grant Activity Report Form to report each educational session or optional activity your organization completed. 

Use the Quarterly Report Form to submit quarterly progress reports to DOH. Reports are due on October 1, 2025, January 1, 2026, April 1, 2026, and June 12, 2026.
