BDAC Trainer Manual (PDF) contains digital formats of all materials located in your hard copy manual.

Tools & Tips for Community Workshops (PDF) to make your sessions more effective and enjoyable.

Sign-in Sheet(PDF) for any BDAC sessions or other activity participation tracking. Save for reporting to DOH.

Name Tag Sheets (PDF) to print to give to participants as they sign in. (Spanish translation)

Training PowerPoint, General PDFs for presentations or distribution. (African American community /Hispanic community)

Red, Yellow, Green Activity Cards (PDF) to use during one of the session's activities.

Evaluation Survey Form (PDF) (Spanish translation (PDF)) to use if participants can’t submit digitally by using a QR code provided in the PPT). If the paper copy is used, trainers will need to submit these to DOH via scanning or mail.

Certificate of Completion (Editable PPT) to distribute to participants if helpful, but not required.

Participant Handout (PDF) to be used by participants throughout the sessions.
