Dial 2-1-1 (https://wa211.org/ ) from anywhere in Washington State and you will reach a highly-trained information and referral specialist who will assess your needs and provide a list of referrals to available resources in your community. Washington 211 has a database of over 27,000 resources to help the people you serve find the right services.


Community-Clinical Linkages

Clinical-community linkages help to connect health care providers, community organizations, and public health agencies so they can improve patients' access to preventive and chronic care services.
