Long COVID Toolkit
About This Toolkit
This toolkit is designed to help health care providers, local health jurisdictions (LHJs) and community-based organizations (CBOs) support their patients and clients with understanding Long COVID. Long COVID is a chronic condition that is often misunderstood and misdiagnosed. These materials offer clear information on its signs, symptoms, and management, helping users more accurately identify the condition and communicate about it with patients and clients.
The toolkit was written and designed with support from providers and people who have lived experiences with Long COVID. It addresses topics such as:
- How to recognize common Long COVID symptoms
- How to have conversations about Long COVID
- How to support someone with Long COVID
- How people living with Long COVID can manage their symptoms
The materials below are separated into two categories: those that are intended for provider, CBO, and LHJ staff education, and those that can be distributed in your offices or handed out to patients and clients.
This toolkit was made possible through a grant allocated from the U.S. Centers for Disease Control and Prevention to the Washington State Department of Health (6NU50CK000515-05-14).

Flyers
These 8½ x 11” educational one-sheets include high-level information about Long COVID. You and your staff can study these to be more informed about:
- What Long COVID is and who it impacts
- Identifying the most common Long COVID signs and symptoms
- Helping patients and clients manage and treat their Long COVID symptoms
Wallet Cards
These wallet-sized cards include a 2 x 3.5" card as well as a foldable 4 x 3.5" card with on-the-go information if you need reminders or tips.
- Symptoms card: Highlights some of the most common Long COVID symptoms
- Conversation starters card: Provides guidance for starting positive conversations about Long COVID
Testimonial Video
Kyla, a provider who is living with Long COVID, shares her experience of having to consult multiple providers before being diagnosed. Kyla’s story can help other providers understand the importance of putting a name to symptoms and having a support system. Posters
These 11 x 17" posters can inform your patients, clients, and community members about the most common symptoms of Long COVID and assure them it's a real chronic condition. These three posters can be placed in lobbies and waiting rooms, as well as exam rooms. Content:
- Highlights chest pains and coughing; for waiting rooms or common areas
- Highlights brain fog; for waiting rooms or common areas
- Offers more in-depth information on various symptoms; for exam rooms
Rack Card
This double-sided 4" x 9" rack card can be handed out to patients or displayed in offices or waiting rooms alongside other educational materials. It highlights common Long COVID symptoms and offers advice for managing them.
Animated Videos
These animated 30-second clips, in English and Spanish, feature a catchy song that explains what having Long COVID feels like. This site is for providers only, please copy the YouTube share link to offer the video to clients.
