Welcome to WA Portal!

To create a new My Portal user account please complete the form with required information below. Make sure to check for typos in your email address. After you click CREATE NEW ACCOUNT an email will be sent to you from (usually within 4 hours). The email will include a one-time login.  Check your spam folder (before requesting assistance) if you do not receive the email within 24 hours. You may need to add this address to your safe senders folder. Thanks for joining the collaboration!

Please be aware that, although My Portal provides a password-protected collaboration space, it is not HIPAA compliant.

A valid email address. All emails from the system will be sent to this address. The email address is not made public and will only be used if you wish to receive a new password or wish to receive certain news or notifications by email.
Credentials will be listed after your name so please enter them as you would like them to be shown.
Enter the name of your primary organization or practice.
Organization Type
Please enter your job title.
Select a role that is most representative of your job title or responsibilities. Use the "Other" box if your role is not represented in the list.
