Who We Are
The Washington State Managed Care Organization Collaborative Performance Improvement Project Workgroup and state Department of Health have come together to improve the Follow-Up After Hospitalization for Mental Illness HEDIS rate for Medicaid enrollees in Washington state. This measure addresses the percentage of inpatient discharges for a diagnosis of mental illness or intentional self-harm among patients ages 18 to 64 that resulted in follow-up care with a mental health provider within seven days.
