Healthcare Provider Data Collection for HPSA Designations
Our Primary Care Office oversees the surveying process of all HPSA eligible providers in Washington. This is to ensure the most recent provider data is in the system to confirm that current HPSA designations still meet federal eligibility requirements when the national updates occur.

What information is collected during this survey?
The survey collects basic demographic information, along with information relating to practice hours, patient population, as well as participation status in federal/state programs.
