COVID Vaccine Program Planning and Response data surveys – due August 23

Recently, some local health jurisdictions (LHJ) received two vaccination-related data requests from DOH’s COVID Vaccine Program Planning and Response team: Agricultural and Food Production Workers and People Experiencing Homelessness. The surveys were sent by email to identified LHJ contacts on August, 11, 2021. The surveys are open through Monday, August 23.

Data collected in the surveys are reported to state leaders to track progress on vaccinating agricultural and food production workers, and people experiencing homelessness. Those who received the surveys by email can also access them here:
