DOH sending Consolidated Contract amendments to local health jurisdictions

This week DOH's Office of Financial Services began sending Consolidated Contract amendments to local health jurisdictions (LHJs). Statements of Work associated with this August amendment will be available in your jurisdiction’s folder in the Upload Center. Amendments, including the associated Exhibit B funding allocation documents, will be sent to designated LHJ partners for electronic signature.

You can access your LHJ’s Statements of Work and Consolidated Contract documents in the Upload Center. To request access to OFS Funding Resources SharePoint site, email Finance@doh.wa.gov.
