Bree Collaborative Survey Bank

The Dr. Robert Bree Collaborative was established to give public and private health care stakeholders the opportunity to identify specific ways to improve health care quality, outcomes, and affordability in Washington State.

This collection of surveys was created to serve as a resource for public health and other professionals looking to evaluate their implementation project(s). There are surveys for school nurses, patients, public health staff, community health centers, and more.
