Welcome to the Care Connect Washington resource webpage. This resource page is managed by the Washington State Department of Health.

Care Connect Washington is a partnership between the Department of Health, eight regional hubs, and more than 100 local care coordinators across the state. The Care Connect system strengthens communities by helping people achieve stability and better health. Local care coordinators work with a person to help them apply for programs such as unemployment benefits, subsidized housing or child care, food assistance programs such as SNAP or WIC, Apple Health, and more.

Care Connect was developed early in the pandemic to provide food and other support to people with COVID-19 so they could isolate at home. The program helped more than 100,000 people across the state, providing fresh food kits, medication delivery, housing assistance, and more.

As the program shifts to focus on connecting people to longer term services, Care Connect will be updating this page with resources.
