WA DOH Care Connect |
Care Connect Washington is a partnership between the Department of Health, eight regional hubs, and more than 100 local care coordinators in communities across the state. The Care Connect system strengthens communities by helping people achieve stability and better health. Local care coordinators work with a person to help them apply for programs such as unemployment benefits, subsidized housing or child care, food assistance programs such as SNAP or WIC, Apple Health, and more. Care Connect provides the information people need in their preferred language. |
